Our primary goal is to ensure that individuals with disabilities and the elderly receive the care and support they need to live independent, fulfilling lives.   • Q. How do I apply for funding from the NDIS and choose EESUPPORT?

    Check Your Eligibility

    • Ensure you are under 65 years old, an Australian citizen or permanent resident, and have a permanent and significant disability that affects your daily activities.

    Gather Supporting Information

    • Collect evidence of your disability from health professionals, including documents proving your identity, residency, and the nature of your disability.

    Make an Access Request

    • Call 1800 800 110 or visit your local NDIS office to initiate the application. Alternatively, download and complete the NDIS Access Request Form and submit it with your supporting documentation.

    Complete the Access Request Form

    • Fill out the form and return it to the NDIS along with the required documentation. You can get help from your Local Area Coordinator (LAC) or local NDIA office.

    Submit Your Application

    • Send the completed form and supporting documents via mail, email, or in person to the NDIS.

    Receive Your Access Decision

    • The NDIS will review your application and notify you of their decision. If eligible, you will proceed to the planning stage.

    Planning Meeting

    • An NDIS representative will arrange a planning meeting to discuss your support and funding needs. Here, you will develop your personalized NDIS plan.

    Choose EE Support Services

    • During the planning process, specify that you would like to use EESupport Services as your provider. Ensure your support plan includes the specific services and supports offered by EESupport Services that align with your goals and needs.

    Implement Your Plan

    • Once your plan is approved, EESupport Services will work with you to implement the supports outlined in your plan, ensuring you receive the necessary support to achieve your goals.

    By following these steps, you can successfully apply for NDIS funding and choose EESupport Services as your provider.
